Выборка представлена всеми осуществленными в Центре стратегической терапии в Ареццо терапевтическими вмешательствами за период с января 1987 года по сентябрь 1988 года.
Поскольку Центр является частным учреждением, прошедшие лечение субъекты обратились за помощью самостоятельно и поэтому представляют собой случайную выборку.
Единственной обшей для всех субъектов переменной было обращение за терапией в этот Центр и то. что со всеми ними было проведено терапевтическое вмешательство согласно стратегическом}' подходу в терапии. Всего было рассмотрено 132 случая, хотя за помощью обратились 149 субъектов (17 человек, или 11%, покинули терапию в течение первых трех сессий и поэтому рассматривались не как подвергнутые лечению случаи, а как случаи прерванной терапии-'8.
Выборка представлена 53 мужчинами и 79 женщинами, средний возраст 26 лет (от 8 лет до 71 года). Социальное происхождение и профессиональная деятельность субъектов были самыми разнообразными и неоднородными. Единственное замечание, кажущееся очевидным, относится к тому факту, что все субъекты смогли позволить себе оплату немалого гонорара частному психотерапевтическому Центру, из чего следует заключить, что никто из них не находился в ситуации серьезных экономических трудностей. Представленная количественная оценка результатов применения стратегической терапевтической модели на данной выборке приводит к некоторым заключениям, позволяющим задуматься как о возможностях данной моде-

ли. так и об общих аргументах статистических исследований в психотерапии.
1) Явным образом демонстрируется эффективность данного типа лечения, несомненно высоким является его общий процент успешности. В самом деле, положительный исход лечения был достигнут в 83% случаев. Кроме того, эффективность лечения была еще более высокой в случае некоторых форм расстройств, например, таких, как фобические расстройства (агорафобия и приступы паники), где эффективность терапии достигала 95%.
Если сравнить эти данные с данными об эффективности других форм психотерапии (Andrews, Harvey, 1981; Bergin, 1971; Garfield, 1981; Giles, 1982; Luborsky, 1975; Sirigatti, 1988; Strupp, Hadley. 1979). где положительный результат — в зависимости от исследования и подхода — состав.ляет примерно от 50% до 80%, то очевидно, что эффективность стратегического подхода, отраженная в количественном анализе результатов нашей работы, превышает среднюю. Кроме того, как упоминалось ранее, эффективность стратегического подхода прошла проверку с помощью последующего наблюдения за пациентами в течение года после окончания терапии. 2 I Эффективность лечения сохраняется во времени. Действительно, процент имевших место рецидивов явно низок, и в большинстве случаев достигнутые в конце лечения результаты сохранились вплоть до третьего последующего наблюдения (по истечении одного года), что делает невозможным рецидивы в будущем, кроме того, не было выявлено возникновения замещающих симптомов. Это опровергает предположение о том — по утверждению Гарфилда (1980) принятое многими терапевтами, — что краткосрочная терапия является поверхностной и симптоматической, и поэтому непременно приводит к рецидиву изначальной проблемы (или к смещению симптома) по истечении времени.
3) Тем не менее именно экономичность лечения является наиболее позитивной характеристикой, которая, как нам

кажется, отличает результаты, достигнутые с помощью стратегического подхода от результатов, достигнутых посредством других психотерапевтических подходов. Другими словами, речь идет о затраченном на достижение результата времени и вытекающем из этого соотношения между затратами и принесенной пользой. Несомненно, на уровне экономичности существует значительное различие между терапией, длящейся в среднем 14 сессий, и терапией, длящейся в среднем 835 сессий (приводимая Гарфилдом (1981) средняя длительность психоаналитического лечения согласно психотерапевтическому научно-исследовательскому проекту фонда Меннннгера39). Настоящая же пенность, относящаяся к большей пли меньшей длительности терапии, по нашему мнению, измеряется не столько экономически, сколько экзистенциально: качеством жизни пациента. Пациент приобретает возможность жить лучше, изменяя свою симптоматическую ситуацию в краткие сроки. Например, реальное п конкретное отличие между лечением тяжелой формы агорафобии за 14 сессий (около трех-четырех месяцев) или за 150 сессий (около трех-четырех лет) заключается в том. что в первом случае пациент, свободный от своих проблем, через несколько месяцев начинает жить гораздо лучше, во втором случае он живет, страдая в кошмаре своих проблем примерно на три с половиной года дольше. Это означает, что его жизнь продолжает быть полной проблем более длительное время, в то время как она могла бы стать менее проблематичной за меньший срок. Как и Гарфилд (1980). мы считаем необходимым в интересах реального благосостояния пациента оценить, какой тип вмешательства, помимо своей эффективности, вынуждает субъекта к меньшим затратам как в экономическом, так и в экзистенциальном смысле. Оценив с этой точки зрения конкретную предъявленную проблему, было бы желательно приступить к лечению при помощи тех процедур, которые представляются более экономичными. Если таковые процедуры не действу- 185
ют или являются недостаточными для решения всех проблем пациента, тогда допустимо обращение к процедурам менее экономичным, но более эффективным для решения данной проблемы. Является очевидным, что не все человеческие проблемы могут быть решены в краткие сроки, однако для того, чтобы удостовериться в этом, сначала необходимо попробовать: поэтому более корректной была бы такая методология, когда вначале выясняется, действует ли краткосрочная терапия, после чего, если она не действует или оказывается недостаточной, возможен переход к более длительному лечению.
После гарантии эффективности терапевтического вмешательства его экономичность становится аспектом, заслуживающим большого внимания — согласно профессиональной этике уважения к пришедшему' за помощью человеку и его праву почувствовать себя .лучше как можно быстрее. И даже если для достижения этой цели необходимо использовать «манипулятивные» стратегии пли «доброкачественный обман», как это происходит во многих описанных в данной книге процедурах, тем не менее на основании вышесказанного мы утверждаем, что такое поведение в терапии является наиболее этичным и корректным, поскольку его целью является достижение наиболее быстрого решения проблем пациента, или. другими словами, достижение им благополучия. Следовательно, «манипуляции» и «доброкачественный обман» не являются негуманным орудием пыток, а являются способом, позволяющим обойти сопротивление пациента и привести его к улучшению состояния с возможно меньшими экономическими и экзистенциальными затратами.
